Employee Benefits

  • Legend makes available a comprehensive health, dental, vision, life, and disability insurance program for full-time Employees and, in the case of health dental, and vision coverage, their dependents through the Blue Cross/Blue Shield and Guardian Insurance companies.
  • Participation in the health and dental plans is voluntary, and coverage will become effective on the date the employee enrolls in the plan.

Medical Insurance

  • Legend Employees are offered comprehensive medical coverage from Blue Cross/Blue Shield (
  • Blue Cross/Blue Shield offers a choice between network and out-of-network benefits.
  • Network benefits include $1,000 individual/$3,000 family annual deductible and a $2,500 individual/$7,500 family annual coinsurance stop-loss maximum.
  • Covered services are paid in full after a co-payment or at 80% after satisfaction of the calendar year deductibles.
  • When network providers are utilized, there are no claims to file and no precertification requirements.
  • Care received out-of-network is subject to the same deductible, higher coinsurance stop-loss maximum and co-payments.
  • Prescription drugs are generally paid in full after a co-payment.

Dental Insurance

  • Currently, coverage includes preventive care, basic and major procedures.   Benefits include a $50 individual/$100 family annual deductible with no deductible for preventive services.
  •  Covered services are generally paid at 80% for basic care and 50% for major care.   Benefits are limited to $1,500 per person per year.
  • Orthodontic care is $1,000 maximum per lifetime

401(k) Plan

  • You can elect to save from 1% to 60% of your total compensation each year (up to certain dollar limits prescribed by law) on a pre-tax basis under the 401(k) Plan.
  • Legend will match 100% of the first 6% of your contributions to the 401(k) Plan each year.
  •  Employees are fully vested upon participation in the plan.
